Welcome to Foot Envy. We schedule new patient visits for up to a full hour, longer than the industry standard 15 minutes, so we have time to actually figure out what is going on and build the right plan with you. What to Expect at Your First Visit

A first visit at Foot Envy follows a predictable rhythm. Here is what happens, in order.


1. Check-in (about 10 minutes)

You arrive about 15 minutes before your appointment time so we can verify your information, check your insurance card, and either receive your completed new patient forms or hand you a clipboard. If you downloaded and completed the forms ahead of time, this step takes about 5 minutes.


2. Vitals and short intake (about 5 to 10 minutes)

A member of our team takes you to the exam room and reviews your forms, your reason for the visit, and any specific symptoms. We then hand you off to Dr. Udall.


3. Your consultation with Dr. Udall (30 to 40 minutes)

This is the bulk of your visit. Dr. Udall sits down with you, asks about your symptoms in detail, examines your feet, watches you walk, and looks at your shoes. If you need imaging, we use our in-office digital X-ray during the visit. You will never be rushed through this part. Most patients are surprised by how thorough the exam is.


4. Diagnosis and treatment plan (10 to 15 minutes)

Dr. Udall explains what is going on in plain English, walks through every realistic treatment option starting with the most conservative, and answers your questions. If a treatment can start that day (taping, padding, in-office procedures, ordering custom orthotics), it often does. You leave with a clear plan, written instructions, and a follow-up scheduled if needed.

You can save 15 to 20 minutes at check-in by completing your new patient forms before your visit. Download, print, and fill them out at home, then bring the completed forms to your appointment.

What the forms include

The new patient forms cover everything we need to start your care:

  • Your contact information and emergency contact.
  • Your primary care doctor information.
  • A brief medical history, including current medications, allergies, and any relevant medical conditions.
  • A description of what is going on with your feet right now, including the type of pain and how long it has been an issue.
  • A short lifestyle section about your work, hobbies, and exercise. This helps Dr. Udall tailor recommendations to your actual life.
  • The HIPAA Notice of Privacy Practices acknowledgment. This is a federally required document for all medical practices. You will sign it once at your first visit.
  • The Foot Envy financial policy.
  • An optional consent to receive appointment reminders by text message.
  • Note about text messaging: opting in is voluntary. You can receive care from Foot Envy without enrolling in text messages. If you do enroll, you can reply STOP to any message to opt out at any time.


Prefer to fill them out at the office?

That is fine. Plan to arrive about 30 minutes before your appointment time and we will give you a clipboard.

What to Bring to Your First Visit

A few practical items make your first visit smooth.

  • A photo ID.
  • Your insurance card (or your Medicare card if Medicare is your primary coverage).
  • A list of your current medications, including dosages.
  • A list of any allergies, especially medication allergies.
  • Your primary care doctor's name and clinic.
  • Prior imaging or medical records related to your foot or ankle, if you have them. If those records are at another practice, give us their name and we can request them.
  • The shoes you wear most often. Sounds odd, but the shoes tell us a lot about how you walk, what kind of support you have, and what we may want to recommend.
  • Completed new patient forms if you downloaded them in advance.
  • Your emergency contact name and number.

Insurance and Payment

We accept Medicare, most Medicare Advantage plans, TRICARE, and most major commercial insurance plans. Our team verifies your benefits online with your insurance company before your appointment so you know what your visit will cost. If you have insurance

You provide your insurance information when you schedule, and we verify it before your visit. At check-in we collect any co-pay required by your plan. If your plan has a deductible that has not been met, we may collect a portion of the deductible at the visit. The remaining balance after insurance processing is billed to you. We file your claim with your insurance company as a courtesy.


If you do not have insurance

Our self-pay new patient office visit fee is $150, collected at check-in. Depending on the care Dr. Udall provides during your visit, such as casting for orthotics, an in-office procedure, or imaging, there may be an additional balance at checkout. We always go over all costs with you before any treatment starts. We do not surprise you with charges at the end of the visit.


If your insurance requires a referral

Some insurance plans require a referral from your primary care doctor before you see a specialist. If your plan requires one, getting the referral to us before your appointment is your responsibility. Without a required referral, your insurance may deny the claim and you will be responsible for the charges. If you are not sure whether your plan requires a referral, call us and we can usually find out.


Forms, disability paperwork, and FMLA

If you need disability forms, work accommodation paperwork, or Family Medical Leave Act (FMLA) paperwork completed, there is a $50 fee for each form completion, payable before the paperwork is processed. This covers the doctor's time to review records and complete the forms thoroughly.

Specific Patient Groups


Medicare patients

Medicare and most Medicare Advantage plans cover podiatry care for medically necessary services. Bring your Medicare card and any supplemental insurance card. Coverage for custom orthotics is generally tied to specific conditions, such as diabetes under the therapeutic shoe benefit. We verify benefits before your appointment so there are no surprises.


TRICARE patients

We accept TRICARE for active-duty service members, family members, and retired military. Active-duty referrals from your military treatment facility (MTF) are welcome. Bring your TRICARE card and, if applicable, your referral paperwork from the MTF. Learn more on our flat feet page and our Luke AFB community page about how we work with military patients.


Diabetic patients

Diabetic foot care is one of our areas of focus. If you have diabetes, please bring your most recent A1c result if you have it, your AM blood sugar pattern, and your diabetic care doctor's name. We coordinate with your endocrinologist or primary care doctor when helpful.


Pediatric patients

We see children and teens for foot and ankle issues. A parent or legal guardian must accompany the minor patient and is responsible for charges. Bring the minor patient's insurance card, photo ID for the accompanying adult, and any prior imaging if available.


Second-opinion patients

If you are coming for a second opinion after a recommendation for surgery elsewhere, bring everything you have. Imaging, the surgeon's notes, the recommended procedure name, and any specific questions. Dr. Udall sees second-opinion patients regularly and will give you honest input.

How to Prepare for Your Visit

You do not need to do much to prepare beyond filling out the forms and gathering the items above. A few small things help us help you.


Be ready to walk. Dr. Udall will watch your gait during the exam. Comfortable clothing that lets you walk normally is the right choice.


Wear shoes you wear regularly. Not your "doctor visit" shoes that you only put on once in a while. The shoes you live in tell us a lot.


If your foot or ankle hurts, do not worry about scrubbing it clean. A normal level of grooming is fine.


Bring questions. Most patients have more questions than they remember once they sit down. Writing them down ahead of time helps.


Bring someone if you want to. Family members and caregivers are welcome, especially for older patients or patients managing complex conditions. A second set of ears is helpful.

Frequently Asked Questions

  • How long is a new patient visit?

    Up to a full hour. That extra time is the whole point of how we run the practice. Returning patient visits are typically shorter, between 20 and 30 minutes, depending on the reason for the visit.

  • Can I fill out the new patient forms in advance?

    Yes, and we recommend it. Download the forms and fill them out at home. This typically saves 15 to 20 minutes at check-in.

  • What should I bring to my first visit?

  • What is your cancellation policy?

    We ask for at least 24 hours of notice if you need to reschedule. This lets us offer your appointment slot to another patient who needs care. Repeated late cancellations or no-shows may result in a charge.

  • Do you offer telehealth?

    Most foot and ankle care requires an in-person exam. We can sometimes handle follow-ups, insurance questions, or specific check-ins by phone. Call us and we will let you know if your situation can be handled remotely.

  • How does the HIPAA Notice of Privacy Practices work?

    The HIPAA Notice of Privacy Practices is a federally required document that explains how we use and protect your health information. You will sign an acknowledgment at your first visit.
